I’ll send you a short form by email — please fill it out and send it back to me signed. That way, I can prepare for our consultation in the best possible way.

Please note: the €75 is a deposit.
The final invoice will be issued after the consultation.

The deposit is non-refundable if:

• the appointment is cancelled less than 48 hours in advance
• a new appointment is not scheduled within 10 working days
